What pushes it up is expanding into the closet or adding a separate water closet. Both mean cutting the slab, and in a slab-on-grade house that is concrete work before it is bathroom work.

Cultured marble vanity tops and builder mirrors are the other giveaway of the era, and replacing both is the cheapest visible change in the room.
